What will the whole bill be?

Ask for a written total for starting care and for staying with it. Put the price after any offer beside the opening price.

A medicine price example, with its limits

Self-pay means paying outside insurance. These are medicine price examples; ask for a separate price for the visit or care plan.

An injection pen is the device used for a shot. The amount printed on it identifies the price group. The letters mg mean milligrams, a measure of amount.

On September 26, 2026, semaglutide (Wegovy) injection had an advertised ongoing self-pay price of $349 per month for pens marked 0.25 to 2.4 mg.

Wegovy HD is the name on the pen marked 7.2 mg; its advertised price was $399 per month at that check.

New self-pay patients could pay $199 a month for their first two boxes of Wegovy pens marked 0.25 or 0.5 mg.

Each Wegovy box held four pens and counted as one month’s supply.

That introductory offer had a December 31, 2026 end date.

The maker can change or cancel the self-pay programme.

Ask the pharmacy whether you qualify for the offer. Ask which labelled pen is in your plan, why that version fits your health, and what later boxes will cost.

Write down the parts of the bill

At the September 26, 2026 check, online care offers often billed for monthly membership and charged for medicine separately.

Ask the service you found whether it charges a membership fee. Ask what that pays for, including visits, messages, and later reviews.

Medicine
Ask for the price of each later supply and any limit on the offer.
Care
Ask about visit fees, a monthly fee, and whether later reviews are included.
Tests and delivery
Ask whether tests, shipping, or other charges add to the total.
Leaving the plan
Ask about prepaid periods, cancellation, and refunds before committing.

Insurance needs its own answer

Give your insurer the specific medicine name and the reason for prescribing it. Ask if approval is needed before payment.

Prior authorization is an insurer’s approval step before it agrees to pay.

As of September 26, 2026, Medicare drug-plan payment can depend on the medical use, the plan’s medicine list, and its approval rules.

Could the Medicare Bridge price apply to me?

A copay means the part of a bill the patient pays when the plan covers it.

GLP-1 is the name of the gut hormone these medicines act like.

For eligible people, the temporary Medicare GLP-1 Bridge has a monthly copay of $50, as checked September 26, 2026. July 1, 2026 is the start; December 31, 2027 is the end.

For weight loss, the Bridge includes semaglutide (Wegovy) shots and tablets; semaglutide (Ozempic) is not included for that use.

Under rules checked September 26, 2026, people need either a separate drug plan under Medicare Part D or a Medicare Advantage plan offering both coordinated care and drug coverage.

Ask your plan to confirm whether your type of coverage qualifies.

At the start of treatment, people must be at least 18 and satisfy the programme’s rules for weight and health.

BMI is a number that relates a person’s weight to their height. Ask the prescriber to find yours and check which group below fits your health record.

Weight and health groups

No other listed health problem is needed to meet the weight rule when BMI is 35 or higher.

With BMI 30 or higher, the rules name heart failure with preserved ejection fraction, kidney disease at stage 3a or worse, or hard-to-control high blood pressure.

Preserved ejection fraction means the heart pumps out a usual share of blood with each beat. Ask the prescriber whether your records name that type of heart failure or that kidney-disease stage.

The blood-pressure rule requires either the top reading to be above 140 or the bottom to be above 90 despite two blood-pressure medicines.

With BMI 27 or higher, the rules name a past stroke or heart attack, pre-diabetes, or symptoms from narrowed arteries outside the heart.

Pre-diabetes means raised blood sugar that is not yet diabetes. Ask the prescriber to check the named conditions in your records.

Health conditions that use the usual drug-plan route

Type 2 diabetes follows the usual Medicare drug-plan path; people with it cannot use the Bridge.

Moderate or severe obstructive sleep apnea also follows that path; this condition causes repeated pauses in breathing while asleep.

It also applies to the fatty-liver disease called MASH with moderate or advanced scarring but without cirrhosis.

Cirrhosis means severe liver scarring. Ask the prescriber to check the exact liver diagnosis; an unfamiliar name is a reason to ask for help.

A prescription to lower the chance of major heart or blood-vessel problems also goes through the usual drug plan.

Previous payment and the application

Payment by your Medicare drug plan for a GLP-1 medicine during 2026 means the Bridge is unavailable to you that year.

The Bridge needs a prescription and an approved application, plus an ongoing food and physical activity plan.

Ask the prescriber and your drug plan to check these rules before treating the advertised price as your own.

How the Bridge payments add up

The Bridge’s $50 payments do not go towards your usual Medicare drug plan’s yearly limit for costs you cover yourself.

The Bridge gives no extra help to lower that copay for people on low incomes.

For a full year of monthly supplies, the Bridge payments add up to $600, separate from what you pay under your usual drug plan.

Reaching your usual drug plan’s yearly limit does not remove the Bridge’s $50 monthly payment.

Ask the plan to explain both parts of your possible yearly bill.
